• For over the counter medication, the form, signed by a parent or guardian, must be submitted to the office with the medication. Medication must be in its original wrapper or container.

  
  

  If prescription medication needs to be administered at school, the health care provider must complete his/her section of the form and the parent must also sign. The completed form must be on file in the office before medication is given. Medication must be in its original prescription bottle. If the dosage changes or if the medication is changed, a new form must be completed.
